We accept most insurance plans. We also are preferred providers for most PPO plans.

Payment Options:

We accept Visa, MasterCard, Discover, American Express, ATM cards, cash and personal checks.

For our patients’ convenience we have arranged a payment plan through CareCredit that helps our patients receive their treatment in a timely manner.

Discounts:

We gladly offer pre-payment and next available appointment discounts!

Recently, many of our patients have lost their dental insurance and benefits. If you find yourself in this situation, we still encourage you to continue with needed dental care and oral cancer screenings to maintain your overall health. For this reason, and as a small gesture of appreciation for our loyal patients, we have decided to create our very own in-house dental plan! Enrollment in this program will enable all of our patients to receive high-quality dental care with minimal out of pocket expense and with no third party interference (such as other dental insurance companies). We give you the benefits directly!

Annual Membership Benefits Include:

Annual Benefit Plan Year runs from date of enrollment through the 365th day following.

• Two (2) Basic Cleanings (D1110) –OR– Two (2) Perio Maintenance Visits (D4910)
• Four (4) Dental Examinations per plan year
• All x-rays needed per plan year
• Two (2) applications of Fluoride Foam –OR– Two (2) applications of Fluoride Varnish
• Twenty Percent (20%) discount on services at McDonald Family Dentistry
• NO annual maximum!
• NO waiting periods!
• NO restrictions!
• NO third-party interference (such as insurance companies)!

Annual Membership Fees:

Annual Membership fees are paid in-full at time of enrollment

*Membership fees & Benefit Plans are non-transferable. Your plan is YOUR plan.

*Benefits cannot be shared between patients. Your benefits are YOUR benefits.

*There will be no refunds for partially used benefit plans.

• Adults: $300 (age 15 and older)
• Children: $250.00 (age 14 and under)
• Payment for Services will be due in-full at the time of service
